1. About Our Services
Dana Sweazy Private Care Consulting ("we," "us," or "Dana") provides private care consulting and care-coordination services for families navigating care decisions for a loved one. Our work includes reviewing your intake information, building a personalized Care Binder, and helping you identify and plan for the right care approach for your family.
We are consultants and coordinators. We do not provide direct medical care, diagnose conditions, prescribe treatment, or employ caregivers directly. Our guidance is meant to support — not replace — the advice of qualified medical and legal professionals.
2. Not Medical or Legal Advice
All information, recommendations, and documents we provide are for general informational and planning purposes only. They are not medical advice, legal advice, or a substitute for consultation with a licensed physician, attorney, or other qualified professional.
You remain responsible for all final care, medical, and legal decisions for your loved one. Always consult appropriate professionals before acting on any guidance that affects health, safety, or legal rights.
3. Your Intake Information
When you complete our intake form, you agree to provide information that is accurate, current, and complete to the best of your knowledge. The quality of the Care Binder and recommendations we build depends directly on the accuracy of what you share.
You confirm that you have the authority to share the information provided about the care recipient and any household or family details included in your intake.
4. Confidentiality
We treat everything you share as private and confidential. Your intake information is reviewed by Dana and used solely to build your care plan and Care Binder. We do not sell your personal information. Please review our Privacy Policy for full details on how your information is handled.
